Travel Health Services provides the following:

• Health consultations for travelers
• Travel immunizations and booster shots
• Destination dossiers
• International Certificates of Vaccination (yellow cards).

Mondays, Wednesdays & Thursdays: 8 a.m. - Noon, 1 - 5 p.m.
Tuesdays: 8 a.m. - Noon, 1 - 6 p.m.
Fridays: 8 a.m. - Noon, 1 - 4 p.m.

Southlake Travel Health Services
1400 Main, Suite 340, Southlake

Appointments are required at all locations. Office visit and prescription fees apply. Vaccination costs vary according to vaccines required. For more information and appointments, call 817-321-4707.
